| I hereby agree to be part of the Kaybase panel of market
research respondents. |

| 1. | Kaybase may ask me to participate in different kinds of surveys - over phone, through the Internet, over email, face-to-face at my house or some other location through posted surveys etc. |
| 2. | Kaybase will not ask me to participate in more than 6 surveys in a year |
| 3. | I agree to participate in atleast 2 surveys per year |
| 4. | Kaybase will not ask me to participate in any surveys where the subject matter is highly objectionable |
| 5. | Kaybase will not use my name or contact information for any kind of direct marketing purpose |
| 6. | My participation in this panel will be understood to include my family members as well i.e. in some surveys, one of my family members may be the respondent rather than me |
| 7. | Kaybase guarantees that my children , if below age of 8 will not be interviewed in any fashion without an adult family member being present |
| 8. | Kaybase will notify me about a new survey through SMS or phone call or email |
